Each time you visit your provider, somebody makes a note of it in your medical records. Before electronic medical records, your doctor scribbled notes on a paper chart. That still happens sometimes, but many health care providers now keep electronic records.
SWLA Center for Health Services maintains an electronic copy of your medical records. All of your visits, test results, phone calls, and other health information are recorded and stored in your secure, confidential electronic chart.
You can obtain a copy of your medical records from the Lake Charles location. To obtain a copy of your records you will need the following:
1. You must first fill out a consent form to request your information
2. You will need current identification
Q. How do I obtain my medical records?
A. Patients will need to fill out a Release of Information form.
Q. How much does it cost?
A. Nothing. It is free to SWLA patients.
Q. How long does it take for me to receive my records?
A. Once the release of information is signed, the records will be sent within 24 to 72 hours.
Q. Can my records be faxed or mailed to me?
A. No records can be faxed or mailed. The patient must come back to pick them up.
Q. How do I get my medical records sent to another provider?
A. To get medical records sent to another provider, the patient must fill out a release of information form.
Q. How do I get my child’s shot records?
A. To get your child’s shot records, telephone the SWLA call center and ask for a message to be given to the Health Information Center. 337-439-9983
